Enrolment Application Form

How to enrol your child

Mater Maria Catholic College is a comprehensive Year 7-12 College within the Diocese of Broken Bay.

We strive for excellence in education and welcome all families seeking a faith-based, co-education for their sons and daughters.

Step 3

To submit your application download the ENROLMENT APPLICATION FORM and email your completed application with attached documents to deborah.fitzgerald@dbb.catholic.edu.au.

Please take care to complete all fields (including religion and language background), and provide signatures on page 11. We cannot accept digital signatures at this time.

We require the following documents to support your application: 

  1. A copy of the applicant’s Birth Certificate
  2. A copy of the applicant’s passport and visa or Australian Citizenship Certificate (if not born in Australia)
  3. A copy of the applicant’s Australian Citizenship Certificate or Australian Passport (if both parents born overseas)
  4. A copy of the applicant’s Baptismal and other Sacramental Certificates (catholic or other if applicable)
  5. A copy of the applicant’s Immunisation History Statement (available from Medicare online through myGov)
  6. A copy of the applicant’s most recent School Report
  7. A copy of the applicant’s most recent NAPLAN Report
  8. A copy of any Specialist Reports and / or assessments for speech, hearing, psychologist for educational needs (as indicated on the Enrolment Application Form)
  9. A copy of Family Court Orders (if applicable)
  10. Any other documents you deem relevant to support the application.

Enrolment offers will be made in Term 2 when the students are in Year 5.

  • Currently accepting applications for 2028 and 2029

  • Waiting list for Year 7, 2026 and Year 7, 2027

Current enrolment vacancies

Vacancies may occur from time to time for current Years 7 to 11. When such vacancies occur all applications held on current waiting lists are reviewed. Subject to an interview with the Principal offers may be extended.
